Clinicians, Practitioners, HSE staff, frontline and management, particularly those with a role in the management of services or delivery of care of Older Peoples care in Ireland.
This update provides a synopsis of the achievements and advancement of the work of the National Clinical Programme for Older People (NCPOP) and Integrated Care Programme for Older Persons (ICPOP) over the past 12 months. Substantial progress has been made in advancing the aims of both programmes, to further support and facilitate the uptake of the Specialist Geriatric Model of Care (2012) and align and support the Integrated Care Programme for Older Persons (ICPOP) 10 steps framework, to deliver improved health and social care for older people in Ireland. Highlights include; “frailty at the front door” pathways, “making the change happen” and a move towards “new ways of working” supported by the NCPOP Frailty Education Programme commenced in 2017, to drive better health outcomes and quality of care for frail older adults.
